Prepare changelog for release of v2.5.0 #1123

Summary

Adds details on the 2.5.0 release to the changelog section in the readme, and removes other changelog entries because they exceed WordPress.org's limits, instead pointing at the GitHub Releases page for detailed notes on each version.

Fixes #1104

Changes Made

  • Updated the WordPress.org Readme file to only include information on the most recent release.
  • Added a link to the GitHub releases section for users to access the full changelog history.
