[BBPBGLIB-1133] Remove ElectrodeManager.hoc and LookupTableV2.mod (#128)

## Context Removed ElectrodeManager and LookupTableV2.mod due to their exclusive use in BlueConfig. ## Review * [x] PR description is complete * [x] Coding style (imports, function length, New functions, classes or files) are good * [ ] Unit/Scientific test added * [ ] Updated Readme, in-code, developer documentation
