Code of Conduct for site

Do we have a code of conduct somewhere for the Google Group and GitHub? (Slack has its own). cc @darrell @twelch @tchaddad

I think @rhodges worked on a dei statement for us in the spring....

but the only code of conduct I think we worked on was the one way back for foss4g:

The top part is a cobbled together 'anatomy' of a DEI statement from my research and experience. The middle section is several statements I looked at to learn from (both good and bad). At the end I attempted to follow the process as devised to generate a full draft.
