Revisit rego policy violation properties #279

Violation is used in rego policies to return structured info about policy violation.

Properties that would be useful:

human-friendly error message
provenance (the file name(s) in which problem was detected)
maybe attribute id(s) and group id(s) - note that we might have more than one for collisions/back-compat checks

Currently violation allows to provide only some of them (single attribute and single group name).
