To Moth monitoring form MHL.1136, add document level fields MY.det and MY.detDate.

We could create a new class, something like MY.documentIdentifications.
This would be inline with our schema, since we have also fields like MY.gatheringEvent that is a
document-level subset of reportable gathering fields

Then adjust ETL so that it copies these values to unit level on DW, unless unit has the corresponding field filled in.
