Implement automatic table of contents

Write a directive which can be inserted to create an automatic MediaWiki-style table of contents.

I think this should be just for wiki pages. Thoughts?

Why? It's a generically useful thing; I see no reason you should be unable to use it in regular Pages.

Fair enough. Going to need to implement it so it's not dependent on either wiki or pages... but that should be doable.
